I purchased a Wolftooth GC 49t for my Sramm 11 speed NX system. When it's in the stand ,I can adjust the indexing just fine. But once I'm on the bike, I have missed shifts, slow shifts and such. Is this just something you put up with ,with an extender. Maybe I should switch back to the original 11x42. And just put the extender on for climbing in the mountains

How old is your cable/housing?  Might be worth at least putting a new cable in if it is old(er).

Dunno how the setup on SRAM compares to Shimano, but I tried a Wolftooth on my Shimano setup a couple of years ago.  In order to accommodate it, I had to remove one of the cogs in the middle which left a larger jump between the two cogs on each side of it.  It was the same deal.  It shifted fine on a stand, but on the trails, there were shifting delays, skips and jumps etc.  I tried and tried to get it to work and even tried different cog sizes in the middle to reduce the gap.  In the end however it remained problematic so I removed the Wolftooth, sold it on ebay and opted for an extended range cassette which shifts like butter.   

That was Shimano though so YMMV.  Good luck!

On some of the forums it says to just remove the 11t. But I'm getting sick of screwing with it. So I think I will take it off and try to sell on ebay

Joe F brings up a real good point.  I run a Sunrace CSMX3 (10spd) and love it.  True I don't have the "Big" ring that the 11spd combos offer, but everywhere I ride I never need more than a 32/42 combo.  It shifts great compared to my buddy's extender cog setup (same setup as yours).  Perhaps the sunrace would be worth a shot?
