Cycling Knee Pain | HELP!!

I have been having some knee pain that seems to be specific to cycling (not while running).  I am looking for some advice or suggestions that may help me figure out this issue.   

The Problem:  Selective sharp pain on the outside of my right knee.  It is 90 degrees outward from the knee-cap and slightly lower.
Occurance:    When hill climbing and applying heavy downward force to the pedals, I feel a quick sharp pain described above (per pedal stroke).
The History:    This started during a ride at sherman when I landed from a small jump.  For the rest of the ride, I shook it off and continued to ride.  During my rides to follow, the pain returns with each pedal stroke when I applying alot of downward force to the pedals.  I have tried platform pedals so my foot could pivot on the pedals, but this did not help.  Undecided

Any comments and/or suggested actions would be appreciated!  Hopefully, this turns out to be something simple!

I have heard that taking Glucosamine (sp?) helps with the joints... I have a similar feeling in my knee when I ride a lot, or have a very tough ride.... but Im too lazy to take my own advice that I got from a fellow rider...

Don't play around w/ knee injuries.  See an orthopedist to see if you injured your knee.  After it's ok to ride again, make sure your fit is spot on.  Good Luck!

Could be your Ilio-tibial band.  Have a doc check it out, then have a PT show you some stretches with a foam roller.  Cured mine.  Also, you may want to have a bike fitter check your cleat alignment, too much toe-in or toe-out could cause these symptoms.

A lot of people get knee pain from pedaling in gears too tall for them at too low a cadence. It's not that surprising you notice more while climbing because likely you climb at a lower cadence than you would normally. Get it checked out and try working on a healthy cadence with more "spinning" up hills than mashing.

5 replies in only a few hours!  Smiley  Thanks for all of the info.  I had a feeling that this could have stemmed from bike fit, but it seemed strange for this to pop up on a seemingly routine ride.  I definitely foresee an orthopedist visit in my future...as well as alot more days on the couch dreaming of riding.  Angry  On the bright side, its winter!   

Mike 5 >> The Ilio-tibial band would be my best guess of the culprit.  It connects right to the area where I feel the pain in my knee.

Any recommendations for bike fitting services?  I know specialized will fit you to you bike when buying from bike source etc...but are there others in CLT?

All of the above is waaaaaaaaaaaay better than I have to chime in but ... once you get this pain all straightened out I found something that helps my knees and joints in general for preventative maintenance.  First off I skateboarded for about 10 years and apparently throwing yourself down flights of stairs leads to arthritis.  I was used to the multiple time a week knee pain I had, could tell when it was going to rain, be super cold etc.  My better half is all about natural fixes so when she told me this I (as usual) thought she was slightly insane.  After much pestering I finally agreed to try it for a month.  6 months later I am still thanking her.  2 tablespoons of apple cider vinegar a day.  It HAS to be the organic "from the mother" ACV found at earthfare (probably other places) etc.  It tastes awful but I mix 1 tablespoon in a few ounces of grape juice or V8 in the morning and one at night and you can't taste it.  Seriously I went from gimping around 3+ days a week to having MAYBE 3 sore days in the last 5 months.  Also helps a lot of other crap she could list off for you but the only other thing I remember is it has helped my blood sugar a lot as I am Type 1 diabetic.  So once you get fixed give that a try along with stretching to stay fixed.

It doesn't sound like bike fit is the problem.  You describe it as starting immediately after landing a jump which suggests a traumatic event.  If I had to guess, I would say it's possible you slightly tore your lateral (outer) meniscus.  This is the fluid filled sack that sit's between the femur and tibia and cushions the knee joint.   

Are you having any swelling in the area.  Either way, an ortho-pod is your best bet to have it diagnosed.  An MRI will probably tell you whether the meniscus is torn right away.

Good luck and let us know how it turns out!

BTW Ortho Carolina's Sports Medicine division on Randolph Rd. is where I went to.  They do the Davidson College sports teams and the Panthers and many others.

Dr. Piasecki is who fixed my knee for me.  (Lateral and Medial Meniscus tears, total ACL tear and partial MCL tear).  Good surgeon.  Very meticulous.

+1 for meniscus tear and +1 for going to see an ortho.  If it were something like the IT band, ligament damage, or bursitis, the pain would be present all of the time.

hurt my knee last year about this time of year, non bike related. I went to ortho carolina, they could'nt tell anything without an mri which was going to cost me about $2500 out of pocket. I opted for a cortizone injection and rode it out. Its basicly ok now, I run and ride all the time. I suppose time will tell the whole story. Hopefully yours will turn out fine.

Not yet but I'm going to give it a try.  On the insurance, I'm glad I had it.  My original Dr. bill was over $40K.  After insurance I payed about $7500 which included my PT copays at $50 a visit. Shocked  I quickly learned how to manage my own PT at the gym at Davidson College (Free. Wife works there.)  I'd go back to the Physical Terrorist (as my Ortho called them) about once a month to check progress and get new exercises.  Surgery was 2 years ago.  Knee is pretty much 100% now.

BTW anyone contemplating jumping out of a PU truck with a bed/gate extender on it...think twice!!!

I did get some cool hardware for the trouble, though....

Dr. said he noticed the bone was a bit soft while inserting the screw in the Tibia for the ACL graft.  Wrapped the rest of the graft around the staple and inserted Bob Villa/This Old House style for some extra grip until the bone healed around the graft and screw.  I feel it twinge every now and then and swear I can feel it with my finger but who knows.  It works and I can ride my bike, run and walk so no worries!

And on the Glucosamine supplements.  I took them 2 months prior and 8 months post surgery.  Seemed to help keep the knee lubricated.  I did stop taking it a year ago as it seemed to be contributing to some swelling.  Stopped taking it and the swelling problem went away.  My body has a tendency to not deal with inflammation well (Some say my personality, too! But that's a topic for another discussion. Wink Tongue  )
My knees never gave me problems before either and don't have problems with any other joints.  What I found worked the best for my knee pain is keeping the Quads, Hamstrings, Glutes and Calf muscles strong. Especially the Quads (all four of them).  They do a lot to support the structure of the knee.  Without it, I get medial meniscus and knee cap pain.  With it, no problems.

I almost wish I had been awake to watch the procedure.  Would have been cool to witness and I had a real problem with side effects from the anesthesia. Tongue

I remember your surgery and rehab from reading the forum, but I thought the injury was bike related. There's no glory in that at all!    Smiley Smiley

WOW!  You guys are making me think its not such a good idea to ride mountain bike!!  HAHA

I will post an updates when I figure this out!  Hopefully, it will blow over without much issue with some rest and advice from a doc...but I agree to not take it lightly.  I'm a rather young guy and havn't had any joint issues in the past so my fingers are crossed!   

Oh...and I plan to try the voodoo magic trick with apple cider vinegar.  My dad said he does this to keep cholesterol down.!  Worst case senario, I will build a tolerance for vinegar!   Grin
